Enerji d\u00fczeyi, yeme i\u00e7me al\u0131\u015fkanl\u0131\u011f\u0131, uyku s\u00fcresi, sosyal ya\u015fant\u0131 ve ruhsal durumdaki de\u011fi\u015fimleri g\u00f6zlemlemek \u00e7o\u011fu zaman bir s\u00fcreci gerektirse de&nbsp; i\u00e7inde bulunulan mevsimin dildeki kar\u015f\u0131l\u0131\u011f\u0131 ile ki\u015fide de ani de\u011fi\u015fimlerin ya\u015fand\u0131\u011f\u0131n\u0131 s\u00f6yleyebiliriz. Peki ya nedir bu sonbahar\u0131n insanda i\u00e7 burkan h\u00fczn\u00fc? \u00dczerine \u015fiirlerin yaz\u0131ld\u0131\u011f\u0131, \u015fark\u0131lar\u0131n bestelendi\u011fi, filmlerin \u00e7ekildi\u011fi, resimlerin yap\u0131ld\u0131\u011f\u0131; kendine sanatta da b\u00fcy\u00fck bir yer edinen sonbahar\u0131n h\u00fczn\u00fc; ye\u015filin ve mavinin, kahverengi ve griye d\u00f6n\u00fc\u015f\u00fcm\u00fc, Gregor Samsa (Franz Kafka, D\u00f6n\u00fc\u015f\u00fcm) gibi uyan\u0131\u015flar\u2026<\/p>\n\n\n\n<p>T\u00fcm bu h\u00fczn\u00fcn, ilham\u0131n ve dilde g\u00fczel tezah\u00fcrleri bar\u0131nd\u0131ran sonbahar\u0131n, gelin bir de psikolojik ve fizyolojik a\u00e7\u0131klamas\u0131na yak\u0131ndan bakal\u0131m.&nbsp;&nbsp;<\/p>\n\n\n\n<p>K\u0131\u015f depresyonu (The Winter Blues), mevsimsel depresyon, mevsimsel affektif bozukluk, mevsimsel duygudurum bozuklu\u011fu olarak adland\u0131r\u0131l\u0131yor sonbahar\u0131n bu h\u00fczn\u00fc. Bu kadar \u00e7ok ad\u0131 olmas\u0131 ba\u015fta beni de tela\u015fland\u0131rsa da ilk kez \u201cSeasonal Affective Disorder\u201d olarak 1984 senesinde Rosenthal ve arkada\u015flar\u0131 (A.J. Lewy ve Tom Wehr) taraf\u0131ndan yap\u0131lan ara\u015ft\u0131rmalar sonucu ortaya konuyor. Kendine 1987 senesinde DSM\u2019de yer edinen SAD (bundan sonra b\u00f6yle adland\u0131r\u0131lacakt\u0131r) g\u00fcn\u00fcm\u00fczde san\u0131lan\u0131n aksine maj\u00f6r depresyonun bir alt t\u00fcr\u00fc de\u011fil; bir depresyon t\u00fcr\u00fc olmakla birlikte bir gidi\u015f \u015feklidir. Hipokrat\u2019\u0131n t\u00fcm hastal\u0131klar\u0131n alt\u0131nda mevsimsel bir sebep aramas\u0131 \u00e7ok da mant\u0131ks\u0131z de\u011fil anlayaca\u011f\u0131n\u0131z. Mevsimsel depresyon, bireyin tan\u0131 almaktan ka\u00e7\u0131nd\u0131\u011f\u0131 rahats\u0131zl\u0131klardan bir tanesi. \u201cKapal\u0131 havalar bana g\u00f6re de\u011fil.\u201d diyenler; haydi soka\u011fa \u00e7\u0131k\u0131n, korkmay\u0131n, sizin iyili\u011finizi istiyoruz. Ancak bu hastal\u0131k yaln\u0131zca k\u0131\u015f ve sonbahar aylar\u0131n\u0131 da kapsam\u0131yor. Yaz i\u00e7in de ayn\u0131 depresyon s\u00fcreci m\u00fcmk\u00fcn olmakla birlikte belirtilerde baz\u0131 de\u011fi\u015fiklikler mevcut. Rosenthal, SAD \u00fczerine yapt\u0131\u011f\u0131 ara\u015ft\u0131rmalar ve ortaya koydu\u011fu sonu\u00e7lar do\u011frultusunda, rahats\u0131zl\u0131\u011f\u0131n tedavisi i\u00e7in \u00e7al\u0131\u015fmalar\u0131n\u0131 halen s\u00fcrd\u00fcrmekte ve \u2018yaz depresyonu\u2019 i\u00e7in de \u00e7al\u0131\u015fmalar\u0131na devam etmekte. Ancak gelin \u00f6ncelikle SAD \u2018nin semptomlar\u0131n\u0131 bir inceleyelim.<\/p>\n\n\n\n<ul><li>A\u015f\u0131r\u0131 yemek yeme, i\u015ftahta art\u0131\u015f<\/li><li>Zevk al\u0131nan \u015feylerden art\u0131k zevk alamama<\/li><li>\u00dcz\u00fcnt\u00fcl\u00fc ruh hali<\/li><li>A\u015f\u0131r\u0131 uyku ve uykuya e\u011filim<\/li><li>Enerji kayb\u0131<\/li><li>Sosyal geri \u00e7ekilme<\/li><li>Yorgunluk, halsizlik<\/li><\/ul>\n\n\n\n<p>Yukar\u0131da verilen semptomlar k\u0131\u015f mevsimine ge\u00e7i\u015fte rastlanmakta ancak yaz mevsimi depresyonunda de\u011fi\u015fkenlik g\u00f6stermektedir. Yaz mevsiminde SAD tan\u0131s\u0131 i\u00e7in semptomlar baz\u0131 de\u011fi\u015fkenlikler i\u00e7ermekle birlikte baz\u0131 ek fakt\u00f6rleri de bar\u0131nd\u0131r\u0131yor. Uykusuzluk, cinsel d\u00fcrt\u00fcde art\u0131\u015f, i\u015ftahta azalma ile birlikte kilo kayb\u0131 gibi semptomlar yaz mevsimlerinde SAD i\u00e7in ek olarak aranan semptomlardand\u0131r. DSM-V\u2019e g\u00f6re te\u015fhis ve tan\u0131n\u0131n konulabilmesi i\u00e7in ki\u015finin iki y\u0131l belirli s\u00fcrelerde belirli semptomlar\u0131 g\u00f6stermesi beklenir. Yap\u0131lan ara\u015ft\u0131rmalara g\u00f6re bu depresyonun kad\u0131nlarda g\u00f6r\u00fclme s\u0131kl\u0131\u011f\u0131 erkeklere oranla 4 kat daha fazla. Tan\u0131n\u0131n ilk konuldu\u011fu ya\u015f aral\u0131\u011f\u0131 ise 18 ile 30\u2019lu ya\u015flar\u0131 kaps\u0131yor. Elbette bunda SAD ile ortaya \u00e7\u0131kan di\u011fer fizyolojik ve psikolojik rahats\u0131zl\u0131klar\u0131n tan\u0131 koymaya engel te\u015fkil etmesinin de etkisi oldu\u011funu belirtmekte fayda var.<\/p>\n\n\n\n<p>Peki bu depresyonun sebepleri nelerdir diye sordu\u011fumuzda ald\u0131\u011f\u0131m\u0131z cevaplar&nbsp; fraktallar gibi farkl\u0131 dallara ayr\u0131lmakta ve her b\u00f6l\u00fcm i\u00e7in farkl\u0131 cevaplar verilmekte. SAD\u2019yi ortaya koyan Rosenthal ve bu alanda ara\u015ft\u0131rma yapan, yapmaya devam eden ara\u015ft\u0131rmac\u0131lar i\u00e7in cevap; k\u0131\u015f ve yaz mevsimi ile birlikte g\u00fcnl\u00fck g\u00fcne\u015f \u0131\u015f\u0131\u011f\u0131na maruz kalma s\u00fcrelerindeki de\u011fi\u015fimleri ve bununla birlikte salg\u0131lanan hormonlardaki de\u011fi\u015fimleri i\u00e7eriyor. G\u00fcn \u0131\u015f\u0131\u011f\u0131na maruz kalma s\u00fcresinin giderek d\u00fc\u015fmesi ile birlikte melatonin sal\u0131n\u0131m\u0131 art\u0131yor ve bireyde daha fazla uykulu hissetme ve enerji d\u00fc\u015f\u00fckl\u00fc\u011f\u00fc g\u00f6r\u00fcl\u00fcyor. Ayn\u0131 zamanda ailenin ge\u00e7mi\u015findeki depresyon tan\u0131s\u0131, ki\u015finin sosyal ya\u015fant\u0131s\u0131 ve maruz kald\u0131\u011f\u0131 stres d\u00fczeyi de etkili fakt\u00f6rler aras\u0131nda. Biyolojik a\u00e7\u0131dan ele ald\u0131\u011f\u0131m\u0131zda retinan\u0131n fark\u0131nda olarak beyne iletti\u011fi \u0131\u015f\u0131k miktar\u0131ndaki de\u011fi\u015fimler bir zaman sonra beyin taraf\u0131ndan fark edilemez hale geliyor ve sirkadiyen ritimde (biyolojik ritim\/saat) bozulmalar ya\u015fan\u0131yor. Ki\u015fi fizyolojik olarak uykuya ihtiyac\u0131 oldu\u011funu farketmeden g\u00fcnl\u00fck rutinine devam ediyor ve bu hali normalle\u015ftiriyor. K\u0131\u015f mevsiminde ya\u011fmur damlalar\u0131n\u0131n cama \u00e7arparak \u00e7\u0131kard\u0131\u011f\u0131 sesi dinleyerek s\u0131cac\u0131k yorgan\u0131n alt\u0131nda uyumak paha bi\u00e7ilemez olsa da g\u00fcnl\u00fck ya\u015fam rutini i\u00e7inde birey, sorumluluklar\u0131n\u0131 yerine getirmekle y\u00fck\u00fcml\u00fc. Hangimiz ekmedik dersi \u00e7\u0131lg\u0131nlar gibi\u2026 Bu s\u00f6z asla s\u00f6ylenmemi\u015f ve bu olay asla ya\u015fanmam\u0131\u015f gibi devam edelim ancak unutmamak gerekir ki SAD, depresyon ve kronik yorgunluk ile olduk\u00e7a fazla kar\u0131\u015ft\u0131r\u0131l\u0131yor. Bunun \u00e7\u00f6z\u00fcm\u00fc de yukar\u0131da belirtti\u011fim \u00fczere DSM baz al\u0131narak bireyin iki y\u0131l \u00fcst \u00fcste belirli d\u00f6nemlerde bu semptomlar\u0131 g\u00f6steriyor olmas\u0131 gerekti\u011fi ve en az 60 g\u00fcn bu semptomlar haricinde g\u00fcnl\u00fck ya\u015fam\u0131na devam ettirmesi. Rosenthal SAD i\u00e7in \u00e7al\u0131\u015fmaya G\u00fcney Afrika\u2019dan ABD\u2019ye ta\u015f\u0131nd\u0131\u011f\u0131nda, k\u0131\u015f aylar\u0131nda \u00fcretkenli\u011finin d\u00fc\u015ft\u00fc\u011f\u00fcn\u00fc, ilkbaharda bu durumun normale d\u00f6nd\u00fc\u011f\u00fcn\u00fc fark ederek ba\u015flam\u0131\u015f. Umuyorum ki Kas\u0131m ay\u0131n\u0131 pencere kenar\u0131nda \u201cNovember Rain\u201d (Guns N\u2019 Roses &#8211; November Rain) dinleyerek ge\u00e7irmez ya da ge\u00e7irmek zorunda kalmazs\u0131n\u0131z.&nbsp;<\/p>\n\n\n\n<p>Bir di\u011fer etkili ve s\u0131kl\u0131kla kullan\u0131lan tedavi y\u00f6ntemi ise \u201cfototerapi\u201d. Genellikle sabah saatlerinde uygulan\u0131yor \u00e7\u00fcnk\u00fc ak\u015fam uygulanan \u0131\u015f\u0131k tedavisinin melatonin d\u00fczeyine etkisi ile uyku problemleri yan etki olmaktan \u00e7\u0131k\u0131yor. Ayn\u0131 y\u00f6ntem uyku problemi ya\u015fayan hastalar i\u00e7in de kullan\u0131lmakta. D vitamini deste\u011fi ve antidepresanla tedavinin yan\u0131nda psikoterapi de tercih edilen tedavi y\u00f6ntemleri aras\u0131nda. Bireyin g\u00fcn i\u00e7erisinde ya\u015fad\u0131\u011f\u0131 karma\u015f\u0131k problem \u00f6r\u00fcnt\u00fclerinin \u00fcstesinden gelmesini ama\u00e7layan psikoterapi, daha az oranda semptom bar\u0131nd\u0131ran hastalarda uygulan\u0131yor. Bireyin SAD s\u00fcresince \u00fczerine d\u00fc\u015fen sorumluluklar\u0131ndan bahsetmemek olmaz. Gece; evin iyi \u0131\u015f\u0131kland\u0131r\u0131lm\u0131\u015f olmas\u0131, g\u00fcnd\u00fcz ise g\u00fcne\u015f g\u00f6ren k\u00f6\u015fesinden ayr\u0131lmamak \u00f6nemli fakt\u00f6rlerden bir tanesi. Elbette g\u00fcnd\u00fcz, g\u00fczel bir rotada \u00e7\u0131k\u0131lan y\u00fcr\u00fcy\u00fc\u015f\u00fcn g\u00fczelli\u011finden ve \u00f6neminden bahsetmeye gerek g\u00f6rm\u00fcyorum. E\u011fer yaz\u0131ma denk geldi\u011finiz vakit ge\u00e7 ise telefonunuzdan ya da bilgisayar\u0131n\u0131zdan farketmeksizin Night Shift (gece modu)\u2018i a\u00e7\u0131n.&nbsp;<\/p>\n\n\n\n<p>Evrimsel olarak Night Shift siz istemeseniz de uykunuzu getirecektir.
